SPECIFIC REQUIREMENTS TO OBTAIN
A LIBRARY CARD
(Updated January 2011)

Valid NJ Driver's License or non-driver's ID showing a current Teaneck street address (no P. O. box) OR

TWO of the following containing the applicant's name and street address:

• Deed.
• Mortgage statement
• Town tax bill
• Lease
• Utility bill (e.g. PSE&G, water, phone)
• Bank statement
• Credit card bill
• Notarized affidavit of landlord
• Letter from shelter or adult residence

Minors, 13 and older, if not accompanied by a parent or guardian with the required ID, may present their most recent report card, class schedule or progress report showing their name and Teaneck address.

COURTESY CARD (TEANECK USE ONLY):

• Letter from employer on letterhead showing employment in Teaneck OR latest pay stub showing name and address of employer

AND

• Driver's license with home address

Note: Library cards are not issued, replaced, nor renewed on Sundays.
